The United Nations Office for Project Services (UNOPS) is an operational arm of the United Nations that provides project management, procurement, and infrastructure support. Through its project services — including infrastructure, procurement, project management, human resources, and financial management services — UNOPS supports governments, the United Nations, and other partners in achieving Member States’ Global Goals, and local objectives for people and countries.
The Health Information and Quality Improvement Project (HIQIP) is a major initiative implemented by the Ministry of Health as the Principal Recipient of a grant from the Federal Government of Germany and managed by the Global Fund to Fight AIDS, Tuberculosis and Malaria (GFATM). The project focuses on strengthening health systems through integration and interoperability of health data systems, enhancing TB screening facilities, and building institutional capacity to improve the quality of healthcare services.
In 2022, the Ministry of Health of Sri Lanka, through the World Health Organization and Global Fund Support, commissioned the development of a National Digital Health Blueprint, Interoperability Plan, Capacity Development Plan and Procurement Plan. These plans were subsequently developed, along with a Roadmap for the execution of the National Digital Health Blueprint.
Considering the extensive nature of the National Digital Health Blueprint, a prioritization exercise has been undertaken to identify a ‘thin-slice' of it to be implemented during the HiQi / D2H project.
The purpose of this assignment is to hire a service provider for the Upgrade of the Hospital Health Information Management System (HHIMS) and Seamless Integration with the National Digital Health Platform (DHP) Components and National Electronic Health Record (NEHR) and Deployment in 30 HiQi Project Hospitals to ensure the successful and timely implementation of the Digital Health Platform (DHP) under the HiQi / D2H project.

New clarification added: Clarification No: 17 (cont.)6. Team & StaffingQuestion 6.1 Section 7.2 (Page 17) The RFP states "The proposed team (key personnel) shall be deployed in Sri Lanka." Does this mandate 100% physical on-site presence at the MoH/hospitals, or is a hybrid/remote working model acceptable as long as the personnel are based within Sri Lanka and available for required physical meetings?Response: The Team should be available in Sri Lanka. The team is not expected to be presence at MoH/hospitals 100%. The offorer is expected to provide work space for the team, refer Schedule of Requirement, section 6.2.Question 6.2 Section 7.1, Item 4 (Page 16) "Overlapping or concurrent project periods will not be double-counted." Does this restriction apply to the firm’s corporate experience, the individual key personnel's CV experience, or both?Response: Schedule of Requirement, section 7.1 Qualifications of the firm refers to the experience of the firm only.Question 6.3 Section 14 (Page 28) "Key personnel designated for 'full-time' roles are unique to each DHP component/bundle." Will Bond 01 be released immediately upon the successful submission and acceptance of Bond 02 and project closeout, ensuring the vendor does not have 20% of the contract value locked simultaneously?Response: Bond 1 shall remain valid for a period of twelve (12) months. Upon completion of all deliverables and submission of Bond 2, the final payment will be released, and Bond 1 shall be released accordingly.Question 7.2 Section 11 (Page 26) "Financial bids in Sri Lankan Rupees (LKR)". Given the potential volatility of currency, does the contract include a price adjustment/exchange rate variation clause for the imported components/services, or is it a fixed firm price in LKR for the entirety of the 30 weeks + 12 months warranty?Response: The proposal currency is LKR. Project Timelines, Acceptance & Payment TermsQuestion 4.1 Section 13 (Page 27) Payment Milestone 5 (15%) is tied to "full deployment of the final version... across all 30 identified hospitals". If deployment at specific hospitals is delayed due to factors beyond the vendor's control (e.g., hardware unavailability, civil works, network outages), will UNOPS allow partial sign-off and pro-rated invoicing for the completed hospitals?Response: UNOPS cannot commit for any unforeseen delays at this stage.Question 4.2 Section 9.2 (Pages 24-26) The timeline requires the MVP at Week 10, Connectathon at Week 14, National Systems Integration at Week 18, and Full 30-hospital deployment by Week 20. This allows only 2 weeks to deploy across 30 hospitals. Is it permissible to stagger the 30-hospital deployment in batches extending up to Week 26 (UAT phase) to ensure operational stability?Response: As payments are tightly coupled with the deliverable shedule stated. It is advisable to adhere it it.Question 4.3 Section 8 (Page 26) "Validation: Requires validation by the Ministry of Health... Technical Assistance representatives, and/or technical steering committee." Will there be a defined SLA for the MoH/Committee to review and sign off on deliverables (e.g., 5 working days)?Response: No5. Intellectual Property & Post-Implementation SupportQuestion 5.1 Section 5 (Page 14) The RFP prohibits vendor lock-in and mandates MoH IP ownership. If the vendor utilizes pre-existing, open-source frameworks, accelerators, or libraries (e.g., standard FHIR transformation engines) that are publicly available under MIT/Apache licenses, is this acceptable provided we do not claim proprietary IP over the final customized implementation?Response: Permisive Open source licenses are encouraged for Mature System enabling rapid implemention.Question 5.2 Section 8 (Page 13) The warranty requires a dedicated capacity of 20 person-days per quarter for enhancements. If the MoH does not utilize the 20 days within a specific quarter, do those days expire, or do they roll over to the next quarter?Response: Not expected to carry forward for the next quarterQuestion 5.3 Section 8 (Page 13) Severity 1 issues require a 4-hour target resolution (24/7). Technical Stack & Architecture (Crucial for Scoping)Question 1.1: Annex 3a & 3b (Pages 38-42) Could UNOPS/MoH provide bidders with read-only access to a repository of the existing HHIMS codebase?Response: The HHIMS codebase cannot be shared during the procurement phase. It will be provided to the successful bidder only. However, we have provided summary of the code structure in the annexures.Question 1.2 Annex 3a & 3b (Pages 38-42) The RFP states the existing HHIMS relies on CodeIgniter 2.x and PHP 7.0.33, both of which are end-of-life and pose significant security/compliance risks. Does the "Upgrade" scope require the vendor to fully migrate/refactor the core platform to a modern, supported stack (e.g., CodeIgniter 4 or Laravel, PHP 8+), or does MoH expect us to patch the existing CI 2.x codebase and simply build integration APIs on top of it?Response: Yes. Scope include the upgrade of the tech stack to the latest PHP and CodeIgniter stack to support the securing of SLCERT certification as well.Question 1.3 Section 4, Item 2 (Page 11) Regarding the "fullSTAC approach" and "configurable privacy controls": If the underlying legacy HHIMS does not natively support these granular privacy controls (RBAC/ABAC at the data level), is the vendor expected to redesign the database schema and application security layer to achieve this?Response: Current HHIMS support role base access controls and further improvements and refinements can be expected.Question 1.4 Annex 2, TS-02 (Page 34) The requirement mandates operating on a "standardised, MoH-approved technology stack." Could UNOPS/MoH kindly provide the exact target technology stack (OS version, database engine/version, PHP version) that is currently approved, so we can ensure our MVP targets these precise versions?Response: The solutions will be deployed in a Proxmox virtualised enviornment with the latest linux based operating system designated by the MoH. Latest Apache, MariaDB, PHP stack is to be used with CodeIgnitor (Ver 4) Question 1.5 Section 4, Item 7 (Page 12) The RFP mentions, "For 10 hospitals with 2 servers each the setup should be configured for high availability (HA)." Does the MoH expect the vendor to provide the HA software licenses (e.g., load balancers, clustering tools) or will open-source HA tools (e.g., HAProxy, Galera Cluster) be acceptable?Response: The virtualised Proxmox setup is intended to be in a open-source HA setup.2. Integration & Dependencies (DHP Components)Question 2.1 Section 3, Page 10 & Annex 1 The RFP lists multiple DHP components (NEHR, NHDX, Registries, APM, SIEM, SLUDI) for integration. Since these are being procured under different bundles, what is their current readiness status? If these national platforms are delayed, will the vendor be allowed to use Mock APIs for the Connectathon and UAT to achieve milestone sign-offs and payments?Response: If the other DHP components are delayed due any unforeseen stituation, Mock APis for Connectathons and UAT can be considered. Question 2.2 Annex 4, CC.INT.02 (Page 47) "The solution SHALL use the national Certificate Services to enable secure communication." Are the national Certificate Services currently operational? If not, should we assume the use of standard self-signed/Let's Encrypt certificates for the MVP phase?Response: If the National implemention is delayed, Let's Encrypt certificates for the MVP phase can be considered. Question 2.3 Section 4, Item 3 (Page 11) "Adopt HL7 FHIR and IHE profiles... Automated standards-based interoperability testing." Will the MoH provide a centralized FHIR validation server and test sandbox, or is the vendor required to build and host the interoperability test harnesses?Response: MoH does not have a centralised FHIR validation server or test sand box. Each vendor for the bundles is expected to deploy a sandboxing enviornment to facilitate intergation and validation.Question 2.4 Annex 1, Item 5 (Page 29) "Support the different health information systems... to be interoperable." The list includes HRMIS, ePIMS, AMC, etc. Is the vendor of this RFP responsible for building the adapters for all these external systems, or is our responsibility strictly limited to exposing standard FHIR APIs from HHIMS for them to consume?Response: Only FHIR API development and proper documentation and conducting Connectathon is the requirement3. Deployment, Data Migration & InfrastructureQuestion 3.1 Annex 5 (Page 49) Row 15 (Colombo South Teaching Hospital) states "HIMS (Excluded)". Could you clarify what "Excluded" means in this context? Are we exempt from deploying to this site entirely, or is there a specific sub-module excluded?Response: CSTH runs a EMR different from HHIMS, thus excluded from the HHIMS deploymentQuestion 3.2 Annex 5 (Page 49) Row 26 (National Institute of Mental Health) states "(Replace the HIMS)". Does this require full data migration from a proprietary third-party system into HHIMS? If so, will the vendor be provided with data dictionaries and export files from the legacy HIMS?Response: NIMS has only patient demographics and admission data to the hospital. Access to relavent data exports will be facilitated after awarding.Question 3.3 Section 4, Item 7 (Page 12) The RFP states deployment must be done "on the new servers procured and virtualised with the ProxMox hypervisors." Who is responsible for installing and configuring Proxmox on the bare-metal servers? Will the servers be racked, powered, networked, and accessible via VPN prior to our deployment phase?Response: This is be provided by the supplier of the servers. Bidders will be provided operational VMs for HHIMS deployment. Question 3.4 Section 6.1, Item 11 (Page 15) "On-premise servers for the deployment of the HHIMS solution in the selected 30 HiQi hospitals". Does reliable VPN/LGN network connectivity currently exist between all 30 hospitals and the central MoH data center to support real-time NHDX/NEHR data exchange? If a hospital has no internet, how should offline resilience be handled?Response: Redundant internet connectivity is planned. But the integration should plan for limited access scenarios and implement a message queue for NEHR commits to prevent data loss. Question 3.5 Annex 2, DB-02 (Page 34) "Patient, clinical... data SHALL be migrated from legacy systems with full integrity validation." For the hospitals marked as "Expansion" or "Upgrade" in Annex 5, what is the estimated total database size (in GBs) and number of historical records to be migrated?Response: This varies across the institutions. Most data are limited to demographics, admission, discharge, OPD, pharmacy and Lab data. The expected size of the database is probably less than 10GB.

New clarification added: Clarification No: 16Question 1:Please provide the daily liquidated damages rate (% per day) referenced in the Tender Particulars, applied to the original total Contract Price, capped at 10% overall.Response:As set out under tender particulars : Liquidated damages details / Delay damages details - UNOPS will deduct from the Contract price, as liquidated damages, a sum equivalent to the percentage of 0.1% of the original total Contract price for each day of delay until actual delivery or performance, up to a maximum deduction of 10%. Once the maximum is reached, UNOPS may terminate the Contract pursuant to the General Conditions of Contract.Question 2:In Form D: Technical Proposal Form, Section 1 (Points 1.1 to 1.6), should the responses be provided separately by each consortium partner, or can a single consolidated response be submitted on behalf of the entire consortium.Response:A consolidated response can be provided. However the response should include relevant details of each of the partners.

Question 2:The scope we have to cover is comprehenvice and need multiple parties with different experties to come together and prepare a complete solution. We are commited to provide the best solutions to Heath sector of Sri Lanka which is considered having best heathcare standards amongh the peers. To achieve this ojective we need a cordinated effort with multiple parties to corrilate. This effor takes time, and getting the correct technical experties and prepare the documentation related to this with comprehencive attention to detail approch. We are cordinating with multiple parties to get the best solution for your requirement. Therefore we are requesting an at least 2 weeks extension to this bid submission. Kindly grant this extention, for us to provide the required granuality to one of the most important projects in Sri Lanka health care.Question 3:We would like to kindly request a 10‑day extension to the submission timeline, as we are currently completing evaluations and finalizing agreements with our subcontractors to ensure we propose the best possible solution. Additionally, the submission deadline for RFP/2026/61570 is on the 25th and requires extensive technical mapping for major components of the project.
