How many customers/clients do we need in the Performance Statement Form? 3. Is there a form or document we need to notarize?4. For the Manufacturer's Authorization Form, is it okay to follow the Manufacturer's standard form? 5. If there are options in pricing example for VMware software 1 year or 3 years, how do we add in the Price Schedule Form? 6. For Certified/Audited Financial Statements for the last 3 years (2025, 2024 and 2023 depending on availability), can we provide 2024, 2023 and 2022 since our 2025 AFS will be released this April 2026?UNOPS's Responses: 1. Please sign only on any page each returnable forms that we required authorized signature. It is not mandatory to sign on every pages.2. Please kindly share the minimum of 02 customers reference by filling the information as required in our returnable form G: Performance Statement Form3. We need authorization letter of the signatory if they are not the head of the entity. This can be in the standard authorization forms of your company and not mandatory to be a notorized letter.4. Kindly use UNOPS standard template for the Manufacturer's Authorization Form.5. Please insert the additional row on the Price Schedule Form where we will evaluation based on any options we select.6. New clarification added: Question: Under 7.4 Software Requirements Details, 70. Virtualization Platform Software – The Supplier must supply and deliver VMware vSphere Foundation licenses (latest version) for the four HCI nodes (see section 7.3.1) includinga) vSphere Enterprise Plus, vCenter Standard, vSAN Enterprise (100 GiB/core) and other features7b) for two (2) CPUs with each 24 cores totaling 176 cores for all nodes (actual number of cores should be aligned to the off er made under the section 7.3.1 requirements)c) including at least a 3-years subscription period covering basic support, maintenance and updates. VMware no longer offers multi‑year licensing for vSphere Foundation and vSAN. Only 1‑year subscription SKUs exist.
