The QUOTATION FORM (pages 2 to 4) is mandatory to be signed and submitted with the technical and financial offers. If the QUOTATION FORM (pages 2 to 4) is not provided, your submission will be disqualified.

Proposals received after the stipulated date and time will be invalidated.
