To develop a national antibiotic guideline for the Republic of Marshall Islands

Purpose of the Consultancy
- To develop a draft national antibiotic guideline; and
- To facilitate the consultation workshop to finalize the draft guideline.
Background
Antimicrobial resistance (AMR) is now considered as a global health threat that requires urgent collaborative action. High rates of resistance to hospital and community-acquired infections have been reported globally, with some of the highest rates reported in the Western Pacific Region. The Ministry of Health of Republic of Marshall Islands (RMI) has recognized antimicrobial resistance as one of the priority agenda items following the laboratory identification of microorganisms resistant to conventional antibiotics. As part of national response in combatting AMR, a draft action plan on AMR was drafted in which one of the strategic activity is to develop a national antibiotic guideline. The current standard treatment guideline for RMI was developed in 2007 and the section on treatment of infectious diseases needs to be updated using the current sensitivity pattern of the country. In view of this, it is proposed to recruit an expert to develop a national antibiotic guideline for RMI.

Start date: 1/11/2017
End date: 15/12/2017
Number of paid working days: Phase I & Phase II – 5 days desk review and 15 days in country – workshop on 4-5 December 2017.
Work to be performed
Method(s) to carry out the activity
Phase I (Desk review)
- Review existing RMI standard treatment guideline.
- Review the antibiogram produced by laboratory team in Majuro Hospital and also country morbidity and mortality data and other epidemiologic data to identify common infection both in the hospital and community
- Draft an antibiotic guideline for RMI that is country specific, for the treatment of common infections in both hospital and community
- Update antibiotics and antimicrobials list in the RMI Essential Medicine List (EML)
Phase II (Finalization – in country)
- Facilitate discussion through face/face interview and country consultation workshop on the draft antibiotic guideline
- Edit and finalize the antibiotic guideline of RMI based on the feedback/comment during the country consultation workshop and by the deadline set by the workshop participants
- Submit the final guideline and technical report to WHO

- Final technical report with key recommendations

Specific requirements
- Qualifications required:
University degree in medicine with post-graduate degree in public health or epidemiology
- Experience required:
More than 10 years of relevant experience in public health, and infectious diseases; some of which should have been obtained in an international context.
- Skills / Technical skills and knowledge:
Demonstrated expertise in rational drug use by using of antibiotic guideline, reviewing of EML, quality assurance, conducting of situation analysis, public health and AMR. Should also have knowledge on hospital and community acquired infections and ability to identify challenges and provide appropriate solutions. Demonstrated experience in inter-sectoral collaboration and ability to work with other agencies and development partners and good writing skills.

Deliverables
Deliverable(s) to be submitted during and after the period of the assignment including manner of delivery and payment:
1. Draft RMI Antibiotic guideline
2. Recommendation for updates on the RMI EML 2016
3. Consultation workshop presentations and meeting report
4. Policy or procedures for restricted antibiotics or antimicrobials use in RMI
5. Final technical report with key recommendations
Payment will be made in the following instalments:
0% - upon acceptance of countersigned contract;
10% - upon signature of contract and acceptance of work plan: for payment of online communication and interview, printing of guidelines and other reference materials.
40% - upon acceptance of consultation meeting presentations and framework of draft antibiotic guideline: for payment of air ticket and per diem.
50% - upon acceptance of final technical report
Detailed cost and budget breakdown
Considering the contractor level of experience (>10 years), expertise and magnitude of this work, STC pay rates at P3/P4 level will be used as a basis for the professional fee. The breakdown of the estimated cost is provided below:
