Feasibility study for the establishment of dialysis service
Purpose of the APW
The purpose of this APW is to undertake a feasibility and costing study for the introduction of a new dialysis service in Kiribati. This assessment will need to consider options of haemodialysis and peritoneal dialysis, acute and chronic renal patients and issues related to human resources, power, water requirements and waste disposal. This assessment will inform planning on the planned introduction of dialysis in Kiribati
Background
Currently patients needing dialysis are referred overseas from Kiribati to neighbouring countries. These include those needing dialysis for acute renal failure and chronic failure. There is a commitment to establishing a service in Kiribati, and in order to plan well for budgetary, HR and other needs an assessment of the costs/benefits of various approaches is needed, along with an understanding of the infrastructural needs.

Requirements - Work to be performed
Objective 1: The completion of a costing, cost-effectiveness and feasibility study for the establishment of dialysis services in Kiribati
Output 1.1: An analysis of the comparative advantage and disadvantage of haemodialysis and peritoneal dialysis to meet the needs in Kiribati, considering local burden and context
Output 1.2: A costing study for the establishment of a dialysis service in Kiribati, which compares: haemodialysis and peritoneal dialysis (including home dialysis options), provision of treatment only for acute or chronic renal failure and considers establishment costs and annual running costs for the next 5 years. This should include infrastructure needs, consumables (including tests and medications) and HR training and staffing needs.
Output 1.3: An analysis of the feasibility issues that would need addressing including power, water (including testing of quality), waste disposal.
Output 1.4: To discuss draft reports with WHO and Ministry of Health team, and revise reports and analysis based on any feedback received.
Requirements - Planning
Output 1.1 Report and powerpoint summarizing analysis. Maximum 10 page report. Timeline by 15/11/2022
Output 1.2 report and powerpoint, along with detailed excel or other tools used to complete assessment. Costing study should detail all assumptions, sources of data and references used for the analysis. Assumptions should be based on agreement with WHO and Ministry of Health team. Timeline by 15/11/2022
Output 1.3 Feasibility report including recommended actions, developed in discussion with Ministry of Health and WHO. Timeline by 15/11/2022
Output 1.4 Presentation to Ministry of Health team and revised reports. Timeline by 15/11/2022
Inputs
The contractor will
- • Undertake consultations with WHO and Ministry of Health team on planned workplan and to gather background information throughout the process
- • Source data from Kiribati allow the work to be completed, including online searches, informal interview and requests for data from relevant units.
- • Complete assessments using established techniques and drawing on WHO guidance.
Characteristics of the Provider
Team should include those with experience working in LMICs, including if possible worki in Pacific Island countries
Place of assignment
This work may be completed remotely, however travel to Kiribati for part of the assignment is preferred
