Supply of Dental Supplies Tender # T22M-01-2022 This Tender opened for Palastine State (Israel, West Bank, and Gaza Strip Only) • Samples are required . The samples must be submitted in a sealed nylon bag or carton box with typing the tender information (tender's tittle, number and supplier name) to Gaza Procurement Office at the same tender closing date (attention: Ms. Manal Bader Phone #: 00972-593114521, otherwise the offer will not be considered, or at UNRWA Jerusalem warehouse (attention: Mr. Mazen El-Mashni Phone #: 0542325617) at the same tender closing date. Otherwise, the offer will not be considered • Each sample must be numbered, and the supplier has to put all required samples in one box. Supplier name/company should not be placed on each item 6.2.1 Proposed delivery term is DAP, UNRWA Gaza Central Pharmacy within 1-2 weeks from receiving the Purchase order . Any supplier who offers different delivery terms than the proposed one in the solicitation documents should mention this clearly in his offer. 6.2.2 Delivery period is of the essence. 6.2.3 Quantities in the tender are approximate and liable to change and UNRWA is not bound to purchase all quantities stated in the tender. UNRWA has right to purchase partial quantities from more than one supplier as per most suitable the delivery schedule or immediate delivery. 6.2.4 The quantities stated in the tender are estimated over two years' to consumption. UNRWA will enter into two years contract (LTA) extendable for another one year subject to mutual agreement between the parties, Performance Evaluation Report and availability of fund. 6.2.5 It is the responsibility of the supplier who has a license of authorization / distribution and also sole authorized for any item to provide all relevant documents along with the offer. 6.2.6 A performance bond of 10% or $ 10,000 (which is less), of the contract amount for contracts that over than $5,000 will be requested in case of awarding. The provided performance bond must be valid for 30 days after the delivery date. 6.2.7 A bid bond (Bank Cheque or Bank Guarantee) of USD 5,000 with a validity of 120 days is required. Copy of the bid bond must be sent along with the offer (electronically) (PDF format) while the original one should be submitted to UNRWA Gaza Finance Office only from 9 am-12 noon at the same tender closing date (Attention: Mr. Ahmed El-Assadi, phone # 0597920666) or to UNRWA Jerusalem Office (Attention: Mr. Mazen El-Mashni , phone # 0542325617) at the same tender closing time and date too. Otherwise, the offer will not be considered. 6.2.8 Poor performance may result in suspension of supplier’s future partake in UNRWA tendering process and/or debarment of awarding for certain period.
