REQUEST FOR QUOTATION: 22-HCR-MMR-SUP-RFQ-022_ for provision of Medical Services for UNHCR in Yangon, Myanmar

Dear Sir/Madam,

The United Nations High Commissioner for Refugees (hereinafter referred to as UNHCR) is pleased to invite prospective bidders to submit a bid in accordance with the UNHCR General Conditions of Contract for Services and the Schedule of Requirements as set out in this Request for Quotation (RFQ).

The RFQ consists of the following;

1.         Annex A: Terms of Reference (TORs)

2.         Annex B: Financial Offer Form

3.         Annex C: UNHCR Medical Examination Form

4.         Annex C1: Ocular examination form for Drivers

5.         Annex D: List of requested Specialists, Tests, and Vaccinations

6.         Annex E: Volume of Examinations 2018-2021

7.         Annex F: Vendor Registration Form       

8.         Annex G: General Conditions of Contract- Service-2018

9.         Annex H: UN Supplier Code of Conduct

Bidders are required to submit any request for clarification or any question in respect of this RFQ by e-mail to Myanmar Supply Unit at myayasupply@unhcr.org.

The deadline for receipt of questions for clarification is 16:00 hrs. on 24/06/2022. Bidders are requested to keep all questions concise.

We would appreciate receiving your quotation by or before 04/07/2022 – 23:59 hrs. Myanmar Time by e-mail only to Myanmar Tender Box at myayaten@unhcr.org.      

Please be aware of the fact that the e-mail policy employed by UNHCR limits the size of attachments to a maximum of [10] Mb so it may be necessary to send more than one e-mail for the whole submission.

Please indicate in the e-mail subject field:

-           Name of your firm

-           Number of e-mails that are sent (example: 1/2, 2/2)
