Description
1. It is important that you read all the provisions of invitation to bid to ensure that you understand and comply with the UNICEF’s requirement. Note that failure to submit compliant offer may result in invalidation of your offer. 2. The completed and signed Bid Form in Annex B must be submitted. The Bid Form must be signed by a duly authorized representative of the Organization/Company. 3. This tender is applicable for local suppliers registered in Yemen. 4. It is required to provide certified audited financial statement for last 2 years 2021-2020. 5. Quotation form must be completed, signed, and returned to UNICEF with the soft excel sheet in Annex C for the required information that you should submit it along with stamped copy. 6. Commercial offer should include all costs (supplies, handling fee, packing, loading and transportation based on attached items in annex C and D for distribution plan. 7. Packing: all contents of each kit will be included in the handbag with waterproof plastic wrap fully covering the kit. 8. Valid trade license and tax ID/clearance must be provided with submitting documents. 9. Partial offer is not accepted for this tender as the contents should be packaged in one kit by awarded bidder who will be the responsible for packaging as described above (note #7) and the delivery at place based on the distribution plan in Annex D.
