National Vaccine Acceptance Study in Indonesia

UNICEF Indonesia would like to request your proposals for Institutional consultancy to conduct the National Vaccine Acceptance Study in Indonesia. Please find attached the following bidding documents and annexes for your references in submitting proposals.

1. LRPS-2020-9155806
2. Annex I – Term of Reference (TOR) and Evaluation Criteria. The mandatory requirement document required under the evaluation criteria must be submitted in your technical proposal. Any missing mandatory document would result un-eligibility for further evaluation.
3. Annex II – Price Proposal Form
4. Annex III – Supplier Profile Form & Check List to be completed only for bidder that never been contracted with UNICEF and submitted along with supporting document required in the check list.

Your offer comprising of a Technical and Financial Proposal/s, in separate document, should be submitted electronically in PDF format to jakartasupply@unicef.org no later than Sunday, 09 February 2020 @17.00 hours Jakarta Local Time.
