Please find attached the Terms of Reference with the specifications and the required forms to be fulfilled. The QUOTATION FORM (pages 2 to 4) is mandatory to be signed and submitted with the technical and financial offers. If the QUOTATION FORM (pages 2 to 4) is not provided, your submission will be disqualified. 

Your offer comprising of a Technical Proposal, a Price Proposal, and the QUOTATION FORM fulfilled and signed should be emailed separately to PacificSupply@unicef.org and referenced as LRFP-2025-9195639 before 25 February 2025 at 23:59 (Fiji Time) as per below:

Second e-mail subject: LRFP-2025-9195639 - Technical Offer 

Proposals received after the stipulated date and time will be invalidated. It is important that you read all of the provisions of the Request for Proposal to ensure that you understand UNICEF’s requirements and can submit a bid in compliance with them. Note that failure to provide compliant proposals may result in invalidation of your proposal.

IMPORTANT DOCUMENTS FOR NEW UNICEF VENDORS

If the vendor is not yet a UNICEF vendor, please provide:

A) A complete copy of your latest audited financial statements with comparative figures for the

two most recent years; preferably signed by your Company's accounting firm/certified external auditor. These financial statements should be both for your Company, and the consolidated financial statements of the ultimate parent company, if applicable.

  • The financial statements are to include, but not limited to, the following:

       • The Balance Sheet (mandatory)

       • The Income Statement/Profit and Loss Statement (mandatory)

       • Statement of cash flows

       • Statement of changes in shareholders’ equity

       • The report from the external auditor, if available

       • Notes to the financial statements, if available

  • Where possible, please provide an English translation of your financial statements if the documentation is not in English. Note that any financial documentation received is treated in the strictest confidence.

B) A copy of your Company’s certificate of legal registration.

C) Your Company’s UN Global Marketplace (UNGM) registration number. You are requested to register for, at the very least, Basic and Level 1 stages. For registration and instructions on how to kindly refer to the attachment or the UNGM site (www.ungm.org/RegistrationProcess)

D) MDM Vendor registration template file available in Excel should be fulfilled and submitted with the other required documents mentioned above. Please provide for bank proof, a copy of your company bank account statement header or company blank/cancelled cheque.
