Purpose of the contract

To support the development of a Role Delineation Policy for the provision of secondary and tertiary health care services for Vanuatu and a Clinical Services Plan for 2018-2030 setting the development priorities towards achievement of medical and nursing workforce development and training, and procurement of specialised equipment.

Background

The Ministry of Health (Vanuatu) is undertaking a review of their role delineation of hospital and health services that was developed in 2004. This follows the completion of the Health Sector Strategy (HSS) 2017- 2020.  A recommendation of the HSS is to review the Role Delineation Policy as one of the actions required to strengthen service planning and improve resource allocation within the health sector. Equitable access to quality, affordable health services is one of the policy objectives of Vanuatu’s National Sustainable Development Plan (NSDP) 2016 – 2030 under Society Goal 3.

The Ministry’s review of the Role Delineation Policy is being undertaken in two phases. The first phase is focused on defining the primary care services (formal and informal) and the human resources, infrastructure (functional space, utilities and support services) and essential equipment required to deliver these services.  This phase of the review applies to Aid Posts, Dispensaries and Health Centres. 

This Terms of Reference (ToR) is for the second phase of this work to define the secondary and tertiary services and provide accompanying guidelines on workforce and service requirements including the physical infrastructure and essential equipment. This component of the Role Delineation Policy is to apply to hospitals in Vanuatu and will be used in developing a Clinical Service Plan for 2018 – 2030.

Work to be performed

Objective and scope of the consultancy

Objectives of the Consultancy are as follows:

  • To define the role delineation of secondary and tertiary services and the workforce and service requirements to deliver these services.
  • To develop a Clinical Services Plan (CSP) for hospital facilities for the period 2018-2030. The CSP is an operational plan that uses the role delineation to determine the clinical service levels and the necessary core services (including, anaesthetics, operating suite, observation, intensive care, radiology, pathology and pharmacy) to deliver these services safely.  The CSP will also provide the basis for specialist medical and nursing workforce development and training, and procurement of specialised equipment.

The Role Delineation should:

  • Consider physical infrastructure, equipment and the ancillary requirements such as service management and administration, monitoring and supervision and health information collection in assessing the service requirements for provision of secondary and tertiary services.
  • Be developed in line with current policy and contemporary practice. Reference should be made to similar activities undertaken in other neighbouring Pacific Islands.
  • Integrate with the Role Delineation for primary health care to ensure that service requirements are developed to include arrangements between primary, secondary and tertiary health care facilities to assist people’s access to higher levels of care as well as their access to follow-up care.

The CSP should include a plan for implementation with appropriate service provision information and baseline data for the measurement of progress toward targets to support ongoing monitoring.

Expected outcome

The secondary and tertiary health care services provided through hospitals will be clearly defined including the levels of staff, skills and training required to provide these services and the functional space, utilities, supplies and equipment required to deliver these services. The Role Delineation for primary health care services will be integrated with the Role Delineation for secondary and tertiary health care services to ensure that service requirements include arrangements between primary, secondary and tertiary health care facilities to assist people’s access to higher levels of care as well as their access to follow-up care.

The Role Delineation Policy will enable the development of a Clinical Service Plan that will guide implementation towards more equitable delivery of services across Vanuatu. This will be based on evidence of health needs and service utilisation through the Ministry’s health information system, relevant findings of the World Bank’s Community Health Services (CHS) 2016 Expenditure Analysis, Vanuatu and other relevant research to support the determination of priorities for service development.

The consultant will be required to meet with the Ministry of Health on an agreed and regular basis in order to review progress against the Project Plan and Schedule of Deliverables.

The specific outputs of the consultant are to:

  1. Project plan and schedule
  • Develop and deliver a project plan and schedule for achieving deliverables for approval by the Ministry of Health within 2 weeks of contract start.
  1. Develop and document the Role Delineation Policy components for the provision of  secondary and tertiary services
  • Review existing national planning and strategy documents.
  • Collect and analyse existing role delineation documents, researching and referencing similar work undertaken in other developing countries.
  • Identify and develop descriptions of secondary and tertiary services, including clinical and non- clinical support services and the workforce and service requirements to deliver these.
  • Test these descriptions of services and requirements with technical experts, hospital staff, Department of Health (DoH) staff and key development partners.
  • Ensure that service requirements are developed to include arrangements between hospital and primary health care services to assist people’s access to higher levels of care as well as their access to follow-up care.
  1. Develop and document a Clinical Services Plan.
  • Apply the Role Delineation to hospital facilities to develop a Clinical Services Plan, making development priorities towards achievement of medical and nursing workforce development and training, development of functional space and procurement of specialised equipment.
  1. Deliver final reports
  • Develop a final Role Delineation of Hospital and Health Services that integrates the Role Delineation for primary health care services. 
  • Develop a final Clinical Services Plan for 2018 – 2030.
  1. Implementation
  • Develop a plan for implementation of the Clinical Services Plan and include appropriate service provision information and baseline data for the measurement of progress toward targets to support ongoing monitoring
  • Work with DoH staff in hospital facilities to develop patient referral management and monitoring tools based on current configuration plans, referral pathways and any regulations.

Additional notes:

Within the Ministry of Health (MoH), the consultant will be supervised by the Director of Policy, Planning and Corporate Services on a day to day basis. The Ministry of Health will assist the development of the Role Delineation and Clinical Services Plan through establishment of an overarching steering committee/working group, with membership of MoH Executive members and other technical officers, including Directors of Public Health, and Hospitals & Curative Services.

Other Technical Assistance (TA) services currently working with the MoH in the areas of Curative Services, Specialist Services and Biomedical Services will be available for consultation in the development of the Role Delineation and Clinical Services Plan, as will other technical officers within the MoH through consultative processes.

Timeframes for availability of other TA support and Development Partner support mechanisms will be made known to the consultant.

Specific requirements

Qualifications required:   

  • Masters Level – preferably in Health Administration, Health Planning and Policy or similar fields

Experience required:

  • At least 10 years high-level experience in health sector policy and planning, and managing health service delivery, in particular, hospital services
  • Experience applying these skills in international development settings

Skills / Technical skills and knowledge:

  • High-level knowledge of health sector planning, policy development and managing health service delivery
  • Must be able to demonstrate an understanding of the interaction between national and provincial level service provision and management
  • Good communication, negotiation and facilitation skills; Expert writing skills; Ability to meet deadlines

Language requirements:

  • English (Expert: Read - Write - Speak)
  • Knowledge of French and/or Bislama would be an advantage

Place of assignment

The assignment will predominantly take place in Port Vila, Vanuatu, with travel to several provinces. Flexible arrangements can be negotiated for completing deskwork.

Timeline and duration of assignment

The consultant will be required for a period of six (6) months, commencing as soon as possible.

Draft Clinical Services Plan submitted to the Ministry of Health for comment- 
22 weeks from commencement

Final Reports (as per Section D and E) including:

  • Role Delineation (Secondary and Tertiary Care Services and integrated with Primary Care).
  • Clinical Services Plan 2018-2030
  • Implementation Plan with appropriate service provision information and baseline data for the measurement of progress toward targets to support ongoing monitoring.-  24 weeks from commencement

A presentation to be delivered to the Health Sector Strategy Working Group  and two additional forums- Not later than 26 weeks from commencement

*The consultant will produce a monthly report for the working group review in order to highlight progress and barriers to progress against the approved work plan.

The consultant will receive monthly payments upon receipt and approval of these reports. Per diems and travel will be paid for by WHO.

The consultant may at times be required to present to the HSS Working Group or MoH Executive during the monthly meeting when required. The first monthly report will outline in detail the role of the consultancy and a schedule of activities to complete the consultancy services in a timely manner.

Detailed cost breakdown

The contract type is a short-term consultancy (STC). All applicants should nominate their daily rate. WHO will pay flights and daily per diems according to WHO assessed rates in addition to consultancy fees. Consultancy rates will be assessed against the consultant’s experience and capacity to meet the deliverables and will be considered during the selection process.
